Supported living care

Supported Living

Our supported living service is designed for adults who want to live as independently as possible while having access to appropriate support. Support can be shaped around daily routines, personal goals, practical tasks, community participation and individual preferences.

We recognise that supported living is not simply about providing accommodation or assistance with tasks. It is about creating an environment where a person can exercise choice, develop confidence, maintain relationships and participate in everyday life. The level and type of support should reflect the person's assessed needs and agreed outcomes.

We work with service users and relevant professionals to understand what support is required and to establish a respectful working relationship from the beginning.

Domiciliary Care

Domiciliary care provides professional support in a person's own home. For many adults, remaining in a familiar environment can be an important part of maintaining routine, comfort and independence. Our approach focuses on delivering support respectfully while encouraging people to do as much as they can for themselves.

Depending on the person's needs and agreed care arrangements, support may include assistance with daily living, personal care, meal preparation, medication-related support where appropriately commissioned, companionship and other practical activities. Services are tailored rather than assuming that every person requires the same type of assistance.

We aim to communicate clearly with service users, families and relevant professionals so that concerns, changes or emerging needs can be addressed appropriately.

Live-in Care

Live-in care can provide a continuous support presence for adults who need more regular assistance or companionship. The service can be particularly useful where an individual wishes to remain at home while receiving a consistent level of support.

Live-in care requires a strong relationship between the person receiving support and the care professional. We therefore place importance on respectful communication, compatibility, clear expectations and an understanding of the person's routines and preferences.

Our goal is to provide practical and emotional support while preserving privacy, choice and independence. Care arrangements should be reviewed when needs change, and communication between the service user, care team and relevant professionals remains important.

Tailored Support

There is no single model of support that suits every adult. Our tailored support service is intended to respond to individual circumstances, including the person's abilities, preferences, routines, communication style and goals.

We work with individuals and service users to provide responsive and respectful support. We put their unique needs, values, and preferences at the heart of every decision. This principle guides how we communicate, plan support and review what is working.

Where appropriate, we work alongside families, commissioners, social care professionals and other partners to create clear expectations and consistent support. The objective is to help people receive the right support in a way that promotes dignity and wellbeing.
